Palliative radiation treatment used for multiple purposes in a single irradiation field

BMJ Case Rep. 2021 Sep 21;14(9):e244172. doi: 10.1136/bcr-2021-244172.ABSTRACTIn this case report, radiation therapy was performed for bilateral hydronephrosis developed during multiple bone metastases of breast cancer and ileus due to peritoneal dissemination. The patient’s preirradiation creatinine level was 8.2 mg/dL, which decreased by the fourth day after starting irradiation therapy. Creatinine level ultimately decreased to 0.6 mg/dL. Pain due to lumbar spine metastasis alleviated and ileus was resolved, allowing the patient to live at home for approximately 5 weeks. The effect of radiotherapy for bilateral hydronephrosis and gastrointestinal obstruction was rapid and good. Palliative radiation treatment can be used for…

Improving time to palliative care review with predictive modeling in an inpatient adult population: study protocol for a stepped-wedge, pragmatic randomized controlled trial

Trials. 2021 Sep 16;22(1):635. doi: 10.1186/s13063-021-05546-5.ABSTRACTBACKGROUND: Palliative care is a medical specialty centered on improving the quality of life (QOL) of patients with complex or life-threatening illnesses. The need for palliative care is increasing and with that the rigorous testing of triage tools that can be used quickly and reliably to identify patients that may benefit from palliative care.METHODS: To that aim, we will conduct a two-armed stepped-wedge cluster randomized trial rolled out to two inpatient hospitals to evaluate whether a machine learning algorithm accurately identifies patients who may benefit from a comprehensive review by a palliative care specialist and…

When to Test in Biomarker-stratified Therapy of Non-small Cell Lung Cancer – Biomarker-stratified Therapy of Non-small Cell Lung Cancer: When and What to Test?

Pneumologie. 2021 Sep;75(9):641-643. doi: 10.1055/a-1486-9057. Epub 2021 Sep 15.ABSTRACTTherapy of non-small cell lung cancer (NSCLC) should be based on biomarker test results in the palliative setting. To this end, testing of all patients in stage IV and in the future also in the earlier stages will be important. In a conference with the patronage of the German Cancer Society, the question of «reflex testing», i. e. independently of tumor stage, was discussed but not deemed to be acceptable. The current report summarizes the results of the consensus conference and discusses possible paths to efficent biomarker testing in NSCLC.PMID:34525486 | DOI:10.1055/a-1486-9057

Mixed-methods study protocol: do national reporting and learning system medication incidents in palliative care reflect patient and carer concerns about medication management and safety?

BMJ Open. 2021 Sep 13;11(9):e048696. doi: 10.1136/bmjopen-2021-048696.ABSTRACTINTRODUCTION: Approximately 20% of serious safety incidents involving palliative patients relate to medication. These are disproportionately reported when patients are in their usual residence when compared with hospital or hospice. While patient safety incident reporting systems can support professional learning, it is unclear whether these reports encompass patient and carer concerns with palliative medications or interpersonal safety.AIM: To explore and compare perceptions of (un)safe palliative medication management from patient, carer and professional perspectives in community, hospital and hospice settings.METHODS AND ANALYSIS: We will use an innovative mixed-methods study design combining systematic review searching techniques…
