Heart Failure with Reduced Ejection Fraction

Med Clin North Am. 2025 Nov;109(6):1157-1173. doi: 10.1016/j.mcna.2025.04.010. Epub 2025 May 27.ABSTRACTHeart failure (HF) is no longer centered on the failing ventricle. Various salutary treatment discoveries now support substantially improved survival with lesser likelihood for urgent care or hospitalization. Careful assessment of comorbidities and early recognition of symptoms further support the care for patients with HF. Advanced care strategies are effective, but not only includes mechanical circulatory assist and heart transplantation, but also clinical trial participation, palliative care, and hospice. With this contemporary view of HF, failure is supplanted with opportunity. At the patient level, longer healthier lives, in concert…

Impact of palliative care on medication use and medical utilization in patients with advanced cancer

Beijing Da Xue Xue Bao Yi Xue Ban. 2025 Oct 18;57(5):996-1001.ABSTRACTOBJECTIVE: To evaluate the effect of palliative care on drug use, medical service utilization and medical expenditure of patients with advanced cancer.METHODS: A cohort of patients including pal-liative care and standard care was constructed using the medical records of the patients in Peking University Cancer Hospital from 2018 to 2020, and coarsened exact matching was used to match the two groups of patients. The average monthly opioid consumption, hospitalization rate, intensive care unit (ICU) rate and operation rate, and the average monthly total cost were selected to evaluate drug use,…

Barriers and Facilitators for Implementation of Palliative Care for Patients with Heart failure – a Rapid Synthesis of Reviews

Curr Heart Fail Rep. 2025 Oct 24;22(1):31. doi: 10.1007/s11897-025-00715-y.ABSTRACTPURPOSE OF THE REVIEW: While the benefits and need for integrating palliative care (PC) services into heart failure (HF) care have been well recognized, integration has been slow. Currently, only a small proportion of patients with HF utilize PC services, meaning there is a discrepancy between the knowledge about optimal care and the implementation of care. In recent years researchers have attempted to understand the barriers and facilitators of implementing PC models to patients with HF which has been captured in both primary, empirical research and secondary literature review papers. We aimed…

Developing a programme theory of implementing patient-reported outcome measures for older people living with severe frailty: a mixed methods study using the consolidated framework for implementation research

J Patient Rep Outcomes. 2025 Oct 24;9(1):126. doi: 10.1186/s41687-025-00951-9.ABSTRACTBACKGROUND: Older people with severe frailty (OPWSF) have palliative needs but typically do not receive specialist palliative care (SPC). Patient-Reported Outcome Measures (PROMs) may offer valuable means to capture these needs. There is a limited understanding of what to include and how to implement PROMs in settings where this group receive most care. The study aimed to: (1) Critically examine how existing PROMs are currently implemented with OPWSF within a SPC setting. (2) Understand how the items with the PROMs are used (3) Develop a programme theory to determine how PROMs can…

Advanced Heart Failure-Where Do We Go From Here?: Therapeutic Options and Palliative Care

Med Clin North Am. 2025 Nov;109(6):1365-1375. doi: 10.1016/j.mcna.2025.04.002. Epub 2025 Jun 9.ABSTRACTAdvanced heart failure is characterized by persistent symptoms, recurrent hospitalizations, and poor survival. In this cohort, mortality remains high without advanced heart failure therapies. This article provides updates on therapeutic options specific to the advanced heart failure population, including durable mechanical circulatory support (MCS) and heart transplantation (HT), and highlights the important role of palliative care.PMID:41136071 | DOI:10.1016/j.mcna.2025.04.002
