Symptom screening with Targeted Early Palliative care (STEP) versus usual care for patients with advanced cancer: a mixed methods study

Support Care Cancer. 2023 Jun 21;31(7):404. doi: 10.1007/s00520-023-07870-9.ABSTRACTPURPOSE: Although early palliative care is recommended, resource limitations prevent its routine implementation. We report on the preliminary findings of a mixed methods study involving a randomized controlled trial (RCT) of Symptom screening with Targeted Early Palliative care (STEP) and qualitative interviews.METHODS: Adults with advanced solid tumors and an oncologist-estimated prognosis of 6-36 months were randomized to STEP or symptom screening alone. STEP involved symptom screening at each outpatient oncology visit; moderate to severe scores triggered an email to a palliative care nurse, who offered referral to in-person outpatient palliative care. Patient-reported outcomes…

Monitoring sarcopenia with wearable devices: a systematic review protocol

BMJ Open. 2023 Jun 21;13(6):e070507. doi: 10.1136/bmjopen-2022-070507.ABSTRACTINTRODUCTION: Sarcopenia is a highly prevalent muscle dysfunction among older adults and is associated with adverse events. The periodic monitoring enables an early screening of patients at risk and control of the progression of muscle impairment. Wearable devices have been used as clinical support for sarcopenia detection. Therefore, this review aims to identify how wearable devices have been used to screen sarcopenia.METHODS AND ANALYSES: Searches will be conducted from August 2023 on PubMed, CINHAL, Embase, Web of Science and SciELO databases. We will include cross-sectional and/or baseline data from prospective studies reporting the use…

The holistic management of malignant bowel obstruction in women with advanced ovarian cancer at end of life

Br J Nurs. 2023 Jun 22;32(12):550-555. doi: 10.12968/bjon.2023.32.12.550.ABSTRACTBowel obstruction is commonly a pre-terminal event in women with advanced ovarian cancer. Determining the patient’s end-of-life wishes is paramount – because the prognosis for these patients can be short, advanced care planning is key. This case study will explore the management of nausea and vomiting associated with malignant bowel obstruction and demonstrate how a patient’s psychological and social wellbeing is as important as managing the physical symptoms. It will discuss how skilled and effective communication is vital early in the…

Impact of an outpatient palliative care consultation and symptom clusters in terminal patients at a tertiary care center in Pakistan

BMC Palliat Care. 2023 Jun 21;22(1):75. doi: 10.1186/s12904-023-01195-4.ABSTRACTBACKGROUND: Patients with terminal diseases may benefit physically and psychosocially from an outpatient palliative care visit. Palliative care services are limited in Pakistan. An improved understanding of the symptom clusters present in our population is needed. The first outpatient palliative care center in Karachi, Pakistan, was established at our tertiary care institution. The primary aim of this study was to evaluate the impact of a palliative care outpatient consultation on symptom burden in patients with a terminal diagnosis. The secondary aim was to analyze the symptom clusters present in our population.METHODS: Patients with…
