Modified Starnes procedure as a bridge to 2-ventricle repair in neonatal Ebstein's anomaly: A paradigm shift

Multimed Man Cardiothorac Surg. 2021 Nov 24;2021. doi: 10.1510/mmcts.2021.077.ABSTRACTSevere tricuspid valve regurgitation secondary to Ebstein’s anomaly represents several challenges in neonates. It can result in significant respiratory and/or hemodynamic compromise that mandates urgent interventions. When conservative management fails, 2 surgical options are available: tricuspid valve repair or single ventricle palliation. The overall results of neonatal tricuspid valve repair are unsatisfactory especially in sick neonates and those with preoperative hemodynamic instability. Single ventricle palliation utilizing the Starnes procedure with right ventricular exclusion provides a quicker way to improve hemodynamics and allows rapid decompression of the right ventricle but carries the long-term…

Palliative Care in Neurology

Praxis (Bern 1994). 2021 Nov;110(15):897-901. doi: 10.1024/1661-8157/a003786.ABSTRACTPalliative Care in Neurology Abstract. Neurology as a discipline clearly overlaps with palliative care. Nevertheless, an early integration of palliative care accompanying neurological treatment rarely takes place, and there are still misunderstandings with regard to the timing of the inclusion of palliative care and its role beyond pure end-of-life care and hospice care. A further expansion and use of synergies should become an integral part of both disciplines in the coming years, and training should focus on appropriate training, especially for our young medical colleagues.PMID:34814720 | DOI:10.1024/1661-8157/a003786

Digital Palliative Care

Praxis (Bern 1994). 2021 Nov;110(15):851-854. doi: 10.1024/1661-8157/a003784.ABSTRACTDigital Palliative Care Abstract. Palliative care is becoming more and more digital. This article illuminates how digital approaches can help identify patients who qualify for palliative care offers and who wish to make use of them. Digital approaches can be used to monitor patients through apps and wearables, but digital methods are also becoming more important in psychosocial and spiritual support. One case demonstrates the therapeutic use of virtual reality. Work organization is digital, and teaching has also become digital during the corona crisis. In spite of all the advantages, however, the potential risks…

Communicating Prognosis and Expectations in Advanced Disease – A Balancing Act in Practice

Praxis (Bern 1994). 2021 Nov;110(15):861-865. doi: 10.1024/1661-8157/a003777.ABSTRACTCommunicating Prognosis and Expectations in Advanced Disease – A Balancing Act in Practice Abstract. To prepare the end of life, to plan important things, to spend the remaining time of life not only with medical treatments, but to live – these are frequently expressed wishes of people with progressive diseases. Prognostic statements are feared by professionals. At the same time, when talking about what lies ahead for those affected, professionals very often make over-optimistic promises. A broader definition of the term prognosis as well as a careful handling of expectations can help to define…

Symptom burden and functional performance in patients with chronic obstructive pulmonary disease

Appl Nurs Res. 2021 Dec;62:151510. doi: 10.1016/j.apnr.2021.151510. Epub 2021 Oct 6.ABSTRACTBACKGROUND: The burden of COPD is growing, and it is one of the leading disease burdens worldwide. Studies on symptom burden as a multidimensional concept in COPD and functional performance are lacking, and little is known about the effect of multiple symptoms and symptom burden on functional performance.METHODS: A descriptive, cross-sectional correlational survey design was used. The Memorial Symptom Assessment Scale (MSAS) and the Functional Performance Inventory-Short Form (FPI-SF) were used to measure the symptom burden and functional performance. Data were analyzed using descriptive and inferential statistics. Participants (n =…
